  5. Print the completed application. Then, initial page three of the application at the bottom-center of page, and sign page four of the application, twice, at the bottom left-hand side of the page.

  6. At this point you may simply fax us the completed and signed application. Or, you may scan the signed application back to your computer desktop. Then, email completed application here.
