2. Complete the first page of the application with your business and personal information.
3. Contact an MSI representative @ (888) 750-6300 to help you determine the right type of merchant account for your business.
4. Complete the rate and equipment pages of the application as per your conversation with your MSI representative.
5. Print the completed application. Sign page one (1) at the bottom left of the page. Sign and initial page two (2) at bottom left and middle center of the page. Sign page three (3) twice at the bottom center of the page.
6. At this point you may simply fax us the completed and signed
application. Or, you may scan the signed application back to
your computer desktop. Then, email completed application here.
